Membership and registration in the CFGA

The basic requirement for becoming a member of the Česká footgolfová a fotbalgolfová asociace z.s. (CFGA) is filling out this electronic application form. 

Common provisions

1. Submitted application form becomes part of the internal info system of the CFGA.
2. In case the application is not filled out correctly it is not possible to issue a membership in the CFGA. Please make sure that there are no mistakes in your application.
3. Part of the membership is also a membership fee. It is not possible to become a member of the CFGA unless the fee is paid and the application form is submitted.
4. The membership fee for youths is 250,- CZK. Youths are considered people who are 18 or younger in the year of the membership fee payment.
5. The membership fee for senior citizens is 250,- CZK. Senior citizens are considered people who are 60 and older in the year of the membership fee payment.
6. The membership fee for adults is 500,- CZK.
7. You have to pay the membership fee to the account of the ČFGA: 2700730986/2010 (FIO BANKA, a.s.). As a variable symbol use the ID generated by the system in your electronic application.
8. The membership is always valid for a calendar year. To renew your membership you always have to pay the membership fee by 28th February of the given year.
9. After submitting your application and processing the membership fee you will be issued a membership card. The membership card will be send to the address that is on your application form.
